Heart failure is not a prison sentence. Many people with heart failure lead happy, fulfilled lives by paying attention to their bodies and taking care of themselves. Through healthy lifestyle changes and proper medication, you can manage your heart failure and lead an active life.
Our program specializes in education and peer support for those living with heart failure. We aim to equip patients to live a healthy, active life by effectively managing their heart disease. Our support groups provide mutual support and advice to manage the aspects of heart failure that are controllable. By understanding your disease and health, we can help you build routines that result in the highest quality of life.

Since heart failure, like diabetes or arthritis, is a chronic disease, our goal is to manage the disease and its symptoms. Although some days are more difficult than others, staying successful in controlling the disease means not being overwhelmed or discouraged by day-to-day changes. The best way is to notice these changes in signs and symptoms and address them promptly.
We offer a wide variety of resources and information designed for you. Our Heart Failure Education and Awareness group learns about the many simple aspects of heart failure that are easily controlled. Working together, we can better meet your needs and manage your heart failure. Our goal is for you to be comfortable in your daily activities and enjoy life.
