Who do I speak to if I have a complaint?

PVHMC is committed to providing the most compassionate and professional care during your stay. If for any reason you feel that you did not receive the service that PVHMC prides its self in, please contact our Patient Relations Department at 909.865.9585.

How do I get a copy of my Birth Certificate?

PVHMC only provides Birth Announcements, if you need a copy of yours or your childs birth certificate please visit the Los Angeles County Registrars Office. For more information please click here.

What do I need to bring to my appointment?

Thank you for choosing us for your healthcare needs. Here is general information to help you prepare, and ensure you receive the best possible care. Please make sure to bring with you the following items:

  • Insurance card and/or policy
  • Medicare or Medicaid card
  • Social Security numbers of the patient and the insured
  • Driver’s licenses and/or photo ID cards of the patient and the insured
  • Emergency contact information
  • A list of your current medications, over-the-counter and prescription drugs, vitamins and herbals
  • Physician’s orders, if you have any
  • Your Advance Directives (living will, power of attorney, etc.)

Lost and found

If you have lost something, please contact Patient Relations at 909.865.9585. Please call as soon as possible so we can assist you in a timely manner.

Are cellular phones allowed at the hospital?

Cellular phones are generally allowed. However, we ask that you be courteous by not speaking on speaker phone or loudly. If asked to turn off your cell phone, please do so immediately.

Is smoking allowed?

Smoking and/or the use of any tobacco products is not permitted anywhere (except in the designated area) in the hospital or on hospital grounds within 50 feet of any buildings.
